Job summary

CenterWell Senior Primary Care in Wichita, KS, seeks a Nurse Practitioner (NP) or Physician Assistant (PA) to deliver comprehensive senior primary care in a collaborative team setting. You will work with physicians, pharmacists, care coaches, and behavioral health specialists to provide high-quality, value-based care and meaningful patient interactions.

The role is in-person at our Parklane office with a focus on extensive patient time, thorough documentation, and coordination of referrals, DME,

Qualifications

• Master’s Degree in Nursing or PA program with board certification.
• Active, unrestricted NP or PA license in the state of practice.
• Screen for TB if selected for the role.

Responsibilities

• Provide comprehensive care to seniors following clinical standards.
• Collaborate daily with the care team to ensure coordinated, high-quality care.
• Manage referrals, hospital/SNF coordination, DME, and home health services.
• Document care with support from quality-based coders.
• Participate in on-call rotation and support growth plans at the center.
• Experience providing value-based care.

What We’re Looking For

Required

Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.

$123,800 - $170,300 per year

This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
