Job summary

CenterWell Senior Primary Care in Louisiana is seeking a dedicated Primary Care Nurse Practitioner for a PRN (as-needed) role. This zero-hour contract suits providers seeking flexible scheduling while delivering high-quality outpatient care to seniors.

You will work onsite at our Shreveport and Bossier City clinics, reporting to the Associate Medical Director, and collaborate with your care team to maintain medical histories, preventive care, and comprehensive senior health services.

Qualifications

• Master's Degree in Nursing with board certification.
• Active NP license in the state of practice.
• Active DEA license and Medicare/Medicaid provider numbers or ability to obtain.
• TB screening may be required.

Responsibilities

• Provide comprehensive care to seniors following clinical standards.
• Collaborate daily with the care team to ensure coordinated, high-quality care.
• Maintain accurate and up-to-date medical histories and records.
