One Medical in Shoreline, WA is seeking a Nurse Practitioner or Physician Assistant to join our outpatient practice. The PCP will work closely with the medical director to advance care delivery and ongoing practice innovation in a value-based care environment.

Formación

• At least 2 years of primary care experience.
• Must be licensed or able to obtain licensure in Washington state.
• Ability to build relationships and communicate effectively both 1-on-1 and in groups.
• Board certification in Internal or Family Medicine.

Responsabilidades

• Provide best-in-class comprehensive primary care in an outpatient setting.
• Collaborate with the medical director and care team to drive care delivery and practice innovation.
• Engage geriatric patient population with behavioral health models to support care and behavior change.
