Health & Human Services Commission (HHSC) in Texas seeks a PNMT Nurse to evaluate and manage physical and nutritional health needs for individuals served by state-supported living centers. You will collaborate with the Interdisciplinary Team to develop and implement care plans, review hospitalizations, and document PNMT activities.

This role supports direct patient care and policy-driven nursing practice. Ideal candidates are registered nurses with Texas licensure, 1+ year RN experience, and

Qualifications

• Licensed registered nurse in Texas or nursing compact state.
• At least 1 year RN experience; 2+ years preferred.
• Experience with individuals with complex physical and nutritional needs preferred.
• Experience with assessment and documentation of health status.

Responsabilités

• Evaluate and analyze assessment information; provide PNMT recommendations.
• Develop IHCP and PNMT plans; document in PNMT progress notes.
• Collaborate with IDT to integrate PNMT into ISP.
• Participate in PNMT meetings; lead or support PNMT activities.
• Provide direct care as needed and ensure compliance with policies.
