Alameda Health System is seeking a Physical Therapist II for an internal-only role at Highland General Hospital in Oakland, CA. The position supervises PT staff, plans treatment programs, and ensures quality patient care.

Under limited supervision, you will evaluate patients, coordinate with the care team, and contribute to program planning. This is an internal, per diem role with a modest full-time-equivalent status.

Qualifications

• Graduation from an approved school of Physical Therapy.
• Current California Board of Physical Therapy licensure as a Physical Therapist.
• BLS - Basic Life Support Certification.

Responsabilités

• Attends team meetings and contributes to discharge planning.
• Attends, participates, and contributes to in-service educational training.
• Communicates with family, case managers, health insurers, etc. as appropriate.
• Confers with other PTs, aides, and department manager for problem solving and program planning.
• Confers with referring physicians and health care team about patient status.
• Completes required administrative forms for leave, payroll, and statistics.
• Writes clear, concise patient evaluations, progress notes, discharge notes, equipment orders, and insurance forms.
• Gathers data for Continuing Quality Assurance and staff surveys.
• Proficient in SMS computer system.
• Uses safety measures; cleans treatment area after use.
• Administers comprehensive PT evaluations and determines appropriate devices.
• Delegates PT tasks to PT Aide/Assistant.
• Develops and implements a treatment plan using PT procedures and modalities.
• Educates patient and caregivers in self-care, home exercise programs, and safety.
• Leads team responsibilities and schedules to support patient care.
• Orients and supervises new staff and students; evaluates clinical performance.
