Job summary

Chatham Hills Subacute Care Center in New Jersey is seeking an Experienced Physical Therapist to lead patient rehabilitation services within a long-term care setting. The PT will assess referrals, develop and supervise treatment programs, and collaborate with the care team to achieve patient-centered outcomes.

Qualifications

• Must have graduated from an accredited Physical Therapy college or university.
• Must have a valid professional license and registration for state of practice.
• Experience in long-term care or geriatric care preferred.

Responsibilities

• Assess referred patients to determine rehabilitation needs.
• Develop and supervise PT treatment plans and goals.
• Document patient condition and progress in records.
• Educate patients and families on therapy methods.
• Oversee and supervise Physical Therapy Assistants.
• Collaborate with other disciplines for patient-centered care.
• Complete evaluations, progress notes, recertifications and discharge documentation.
• Maintain confidentiality and HIPAA compliance.
