Job summary

Jobtailor seeks a dedicated Physical Therapist to provide rehabilitative services for senior patients within a coordinated care team. You will plan initial and ongoing treatment programs based on clinical findings and collaborate with rehabilitation therapists to ensure cohesive care.

The role includes one-on-one private sessions using a functional-based approach, tailoring plans to individual needs and participating in clinical conferences to align the overall plan of care.

Qualifications

• Degree in area of discipline
• Current Physical Therapy license in state of practice, or ability to obtain prior to beginning employment
• Excellent interpersonal, fiscal, and clinical skills
• Computer proficiency
• Minimum commitment of 8 hours per month to maintain active employment

Responsibilities

• Provide rehabilitative services to patients under specific medical orders as part of a coordinated team
• Plan initial and subsequent treatment programs based on clinical findings
• Participate in clinical conferences to share and coordinate the plan of care for patients
• Treat residents one on one in private treatment sessions using a functional-based treatment approach
• Individualize treatment sessions for senior patients
• Build relationships with patients and collaborate with rehabilitation therapists
