Legacy Healthcare Services is seeking a Physical Therapist to deliver rehabilitation care to senior residents within a team-based setting. You will plan and implement treatment programs, coordinate with clinicians, and provide individualized sessions to promote functional independence.

Qualifications

• Degree in Physical Therapy is required.
• Current Physical Therapy license in state of practice, or ability to obtain prior to starting employment.
• Excellent interpersonal, fiscal, and clinical skills.
• Computer proficiency.

Responsibilities

• Provide rehabilitative services to residents under medical orders as part of a coordinated team.
• Plan initial and subsequent treatment programs based on clinical findings.
• Participate in clinical conferences to coordinate the plan of care for residents.
• Treat residents one-on-one with private treatment sessions and individualized attention.
• Customize treatment sessions to each patient’s needs.
