You will perform initial evaluations, develop individualized plans of care, and provide a range of direct treatments within a team-based setting. Qualifications include five years of clinical PT experience, CA licensure upon hire, and BLS/CPR certification.

Qualifications

• Minimum five years clinical experience in physical therapy.
• CA licensure required upon hire.
• BLS/CPR certification required upon hire.
• Specialty areas may include neuro rehabilitation, orthopedics, pediatrics, cardiac rehabilitation, hand rehabilitation, pain management and work hardening.

Responsibilities

• Perform initial evaluations and develop individualized plans of care.
• Provide direct physical therapy treatments.
• Maintain patient care productivity and collaborate with the multidisciplinary team.
• Identify and report patient abuse/neglect.
• Communicate goals and progress with the care team; attend rounds and meetings.
• Participate in continuing education and in-services; plan for discharge and referrals.
