Job summary

Luxor at Sayville Rehabilitation and Nursing Center seeks an experienced Physical Therapist to lead rehabilitation efforts for residents in a long-term care setting. The role includes direct patient care, assessments, goal-oriented treatment planning under physician direction, and education for patients and families.

Candidates must be licensed, have an accredited PT program, and be comfortable supervising Physical Therapy Assistants while collaborating with a multidisciplinary team to deliver

Qualifications

• Must have graduated from an accredited Physical Therapy college or university.
• Must have a valid professional license and registration for state of practice.
• Experience with long-term care and/or geriatric care is preferred.
• Strong documentation-writing skills.
• Ability to supervise Physical Therapy Assistants.
• Demonstrated patient-centered care and professionalism.

Responsibilities

• Assess referred patients requiring rehabilitation and provide direct patient care.
• Establish and administer treatment programs with goals per physician direction.
• Document patient condition and progress; educate patients and families.
• Supervise and oversee Physical Therapy Assistants.
• Collaborate with all disciplines to provide best patient-centered care.
• Maintain confidentiality and HIPAA compliance.
