Job summary

Luxor at Sayville Rehabilitation and Nursing Center seeks an Experienced Physical Therapist to assess referred patients and implement rehabilitation plans under physician guidance. The role emphasizes patient-centered care, documentation, and education for patients and families.

Candidates must have graduated from an accredited PT program and hold a NY PT license. Experience in long-term care and strong teamwork are highly valued.

Qualifications

• Must have graduated from an accredited Physical Therapy college or university.
• Must have a valid professional license and registration for state of practice.
• Oversee and supervise Physical Therapy Assistants.
• Experience with long-term care and/or geriatric care preferred.
• Well versed with documentation writing.
• Evaluations, weekly progress notes, recertification and discharge documentations.
• Collaborates with all disciplines to provide best patient-centered treatment and care.
• Exceptional patient service skills and ability to demonstrate clinical excellence.
• Report to work on time, adhere to scheduled hours and demonstrate professionalism.

Responsibilities

• Assess referred patients requiring rehabilitation and provide direct care.
• Establish and administer treatment programs with goals aligned to patient capacity.
• Document patient conditions and educate patients and families.
