Job summary

Luxor at Sayville Rehabilitation and Nursing Center seeks an experienced Physical Therapist to provide patient-centered rehabilitation. You will assess referred patients, develop treatment plans, and monitor progress under physician direction.

The role requires licensure, graduation from an accredited PT program, and ability to supervise Physical Therapy Assistants; geriatrics experience is preferred. We offer competitive compensation and opportunities for professional growth.

Qualifications

• Must have graduated from an accredited Physical Therapy program.
• Must have a valid professional license and registration for the state of practice.
• Experience supervising Physical Therapy Assistants.
• Experience with long-term care and/or geriatric care preferred.
• Proficient in documentation writing.
• Experience with Case Mix Index and PPS scheduling preferred.

Responsibilities

• Assess referred patients and provide direct patient care.
• Develop and administer treatment programs with goals based on patient capacity and tolerance.
• Document and record patient condition; educate patients and families.
• Supervise and oversee Physical Therapy Assistants.
• Collaborate with other disciplines to provide patient-centered care.
• Maintain confidentiality and HIPAA compliance.
