Job summary

Sayville Rehabilitation & Nursing Center is seeking an Experienced Physical Therapist for Luxor at our facility. The PT will assess patients in need of rehabilitation, design and implement treatment plans, and educate patients and families.

The role includes supervising Physical Therapy Assistants and coordinating with other disciplines to deliver patient-centered care. Qualifications include an accredited PT degree, active license, and experience in long-term or geriatric care.

Qualifications

• Graduated from an accredited Physical Therapy program.
• Licensed and registered to practice in the state.
• Experience supervising Physical Therapy Assistants.
• Experience in long-term care or geriatric settings preferred.
• Strong documentation and record-keeping abilities.
• Ability to perform evaluations, progress notes, recertifications and discharge summaries.
• Collaborates across disciplines to deliver patient-centered care.
• Demonstrates clinical excellence and professionalism.
• Adheres to confidentiality, HIPAA and Residents' rights.

Responsibilities

• Assess referred patients for rehabilitation needs and medical status.
• Develop and administer treatment plans with physician direction.
• Set realistic goals and educate patients and families.
• Document patient conditions and progress accurately.
• Oversee and supervise Physical Therapy Assistants.
• Coordinate with other disciplines to optimize care.
• Maintain timely, compliant clinical documentation.
