Job summary

Qualifications

• Must have graduated from an accredited Physical Therapy program.
• Must hold a valid PT license/registration for state.
• Supervise Physical Therapy Assistants.
• Experience in long-term care or geriatric settings preferred.
• Strong documentation and reporting skills.
• Collaborate with all disciplines to provide patient-centered care.
• Punctual, professional, and compliant with confidentiality and HIPAA.

Responsibilities

• Assess referred patients to determine rehab needs and goals.
• Develop and administer individualized PT treatment plans.
• Document patient progress, prepare weekly notes and discharge summaries.
• Educate patients and families on rehabilitation methods.
• Coordinate care with other disciplines under Rehabilitation Director.
