Job summary

Water’s Edge Rehab and Nursing Center at Port Jefferson seeks an experienced Physical Therapist to assess referred patients, develop treatment plans, and deliver direct patient care under physician direction. Responsibilities include documenting progress, coordinating with the care team, and supervising Physical Therapy Assistants while ensuring confidentiality and adherence to HIPAA.

Licensure is required; long-term care experience is preferred. Competitive compensation.

Qualifications

• Graduated from an accredited Physical Therapy program.
• Must hold a valid license and registration for the state of practice.
• Experience with long-term/geriatric care preferred.
• Proficient in documentation and record-keeping.

Responsibilities

• Assess referred patients requiring rehabilitation and provide direct patient care.
• Develop and administer treatment plans with goals based on capacity and tolerance under physician direction.
• Document patient conditions and educate patients and families in physical therapy methods.
• Supervise Physical Therapy Assistants and collaborate with all disciplines to provide patient-centered care.
