• Partner with respiratory, PT, and pharmacy to keep the contract care plan moving as one
• Document care interactions in the electronic health record accurately
• Audit the unit's Telehealth compliance weekly and brief the charge Pharmacy Technician on the gaps
• Walk the Hayward, CA floor at handoff, laying eyes on every patient before you sign out
• Coordinate discharge planning alongside case management, social work, and the patient's family in Hayward
