Job summary

Centene Corporation seeks an experienced RN to coordinate care for pediatric members with complex needs in the Sugar Land area. You will assess, develop, and monitor individualized care plans with a multidisciplinary team, educate families, and ensure compliant, high-quality care across services.

The role requires 4–6 years of nursing experience, preferably in pediatric settings, with flexibility to accommodate school schedules and travel for member assessments.

Qualifications

• Requires Graduation from an Accredited School of Nursing and 4–6 years of related experience.
• RN license is required; BSN preferred.
• Experience in Pediatric settings (Acute/Pediatric Home Health/Private Duty) is valued.

Responsibilities

• Evaluate the needs of the most complex or high-risk members and plan the care.
• Develop and adjust long-term care plans with care teams and resources.
• Coordinate care with members, families, and providers to ensure appropriate services.
• Monitor progress and revise plans to meet member goals and needs.
• Educate members and families on care options, services, and benefits.
• Serve as liaison between member/family and healthcare providers.
