The University of Miami's Bascom Palmer Eye Institute in Miami, FL seeks a Patient Navigator to join our full-time staff. You will guide patients through evaluations, help access insurance and social services, and coordinate between patients and providers to ensure timely care.

The role includes reminding patients of appointments, addressing barriers to care, educating patients, and handling medical records and communications while maintaining strict confidentiality. Equal opportunity employer.

Qualifications

• Education: High School Diploma or equivalent/relevant experience, certification or license.
• Experience: Minimum 1 year of relevant experience required.
• Knowledge, Skills and Abilities: Learning Agility, Teamwork, Technical Proficiency, and Communication.

Responsabilités

• Conducts patient evaluations to obtain information necessary for treatment planning.
• Ensures patients access to insurance and social services.
• Provides patient reminders and liaison between patients and providers.
• Identifies and resolves barriers to care.
• Educates and counsels patients on health and life issues.
• Maintains knowledge of community health resources.
• Handles patient communications and medical records requests.
• Addresses care gaps with the nurse navigator.
• Prepares, tracks, and processes medical records and pathology specimens.
• Reviews and files medical records for completeness; uses tracking systems for documentation.
• Provides administrative support to the department: phones, inquiries, messages, copying, faxing, emailing, mailing while ensuring confidentiality.
