Job summary

The Ohio State University Palliative Medicine Team is seeking a Senior Specialty Practice Pharmacist to lead the palliative pharmacy service, oversee staffing, and coordinate onboarding processes.

The pharmacist will perform daily comprehensive medication reviews, optimize symptom control, manage drug interactions, and collaborate with physicians, nurses, and students in both inpatient and outpatient settings. This role emphasizes opioid stewardship and quality improvement activities.

Qualifications

• Bachelor's degree in Pharmacy; active Pharmacist license.
• 1 year of relevant experience required; 2-4 years preferred.
• Ability to screen for drug interactions and manage therapy in complex patients.

Responsibilities

• Provide drug information to patients, caregivers, health care professionals, students, and staff.
• Screen for drug-drug, drug-disease, and other interactions; ensure appropriate dosing and therapy.
• Lead onboarding and supervise Pharm.D. students and pharmacy staff.
• Participate in quality improvement activities and committees relevant to palliative care.
• Collaborate on opioid stewardship to promote safer prescribing.
