Job summary

The Ohio State University Palliative Medicine Team is seeking a Senior Specialty Practice Pharmacist to optimize symptom control through thorough drug therapy management and daily patient reviews in inpatient and outpatient settings.

The successful candidate will assess interactions and laboratory data, collaborate with care teams, and contribute to a robust multidisciplinary practice focused on safe, effective pharmacotherapy for palliative patients.

Qualifications

• 1 year of relevant experience required.
• 2-4 years of relevant experience preferred.

Responsibilities

• medication order approval and distribution; assurance of appropriate dose, route, frequency and duration of therapy; and coordination of patients' pharmaceutical needs with providers and nursing staff.
• Patient population includes adult and geriatric patients; must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served.
• Collaborate with other providers in opioid stewardship to demonstrate less opioid exposure and safer opioid prescribing within the medical center.
• Coordinate all aspects of patients' pharmaceutical needs in the patient care areas.
• Supervising Pharm.D. students, technicians, undergraduate and graduate student teaching, and providing educational programs for hospital staff (providers, nurses, and pharmacy staff).

Bachelor's Degree in Pharmacy
