NJ Palliative Care Solutions is hiring an experienced Nurse Practitioner or Physician Assistant to provide outpatient and SNF-based palliative care across Middlesex & Monmouth counties in New Jersey. The role emphasizes an experienced-provider track with mentorship, clinical backup, and data-guided coaching.

The position features a competitive base salary of $130,000–$160,000 per year, plus an annual performance bonus up to 12.5%, generous PTO, malpractice coverage, license/DEA support, CME, and

Qualifications

• Active New Jersey Nurse Practitioner or Physician Assistant license and national board certification.
• 2+ years of palliative care or closely related serious-illness experience.
• Palliative care certification preferred.
• Strong communication, clinical judgment, and comfort working on interdisciplinary teams.

Responsibilities

• Provide outpatient and SNF-based palliative care across Middlesex & Monmouth.
• Conduct palliative assessments and follow-up visits.
• Lead goals-of-care and advance care planning conversations.
• Coordinate with home health and hospice agencies as appropriate.
• Complete timely documentation aligned with billing and quality standards.
• Travel locally within Middlesex & Monmouth with mileage reimbursement.
