Job summary

NJ Palliative Care Solutions is seeking an experienced Nurse Practitioner or Physician Assistant to provide outpatient and SNF-based palliative care across Central New Jersey for patients living with serious illness. This is our experienced-provider track: 2+ years of palliative, hospice, or other serious-illness care is required.

You'll join an interdisciplinary team with physicians, APPs, social work, care coordination, and operational support, and you will work Monday–Friday with scheduling

Qualifications

• 2+ years of palliative, hospice, or related serious-illness care experience required.
• Active New Jersey NP or PA license and national board certification.
• DEA and CDS registrations or ability to obtain by start date.

Responsibilities

• Provide outpatient and SNF-based palliative care across Central New Jersey.
• Conduct palliative assessments and follow-up visits in SNFs and community settings.
• Lead goals-of-care discussions with patients and families.
