Job summary

NJ Palliative Care Solutions is hiring an experienced Nurse Practitioner or Physician Assistant to deliver outpatient and SNF‑based palliative care across Central New Jersey for patients with serious illness. The role requires 2+ years of palliative care experience and active NJ NP/PA license, with DEA/CDS registration or readiness to obtain.

Qualifications

• Active New Jersey NP or PA license and national board certification.
• DEA + CDS registration, or ability to obtain by start date.
• 2+ years of palliative care or closely related serious‑illness experience.
• Palliative care certification preferred.
• Strong communication and comfort working on interdisciplinary teams.

Responsibilities

• Deliver outpatient and SNF‑based palliative care across Central New Jersey.
• Provide palliative care consultations and follow‑up visits in SNFs and community settings.
• Lead goals‑of‑care conversations and coordinate with home health/hospice as appropriate.
• Complete timely documentation aligned with clinical, billing, and quality standards.
• Travel locally within Central NJ, with mileage reimbursement and EMR support.
