The Senior Outreach Worker identifies isolated or home-bound seniors and provides or locates services that allow them to maintain their independence.

Hourly Rate of Pay: $19.05
High school diploma or general education degree (GED) and, six months of related experience and/or training; or, an equivalent combination of education and experience.
The employee in this position must possess and maintain a valid New York State driver’s license and an acceptable driving record. As a condition of employment, employees in positions requiring driving will be enrolled in the New York State Department of Motor Vehicles License Event Notification Service (LENS) program.
