TRIUNE Health Group in Texas is seeking a Nurse Case Manager to coordinate resources and develop flexible, cost-effective treatment plans for catastrophically or chronically ill or injured individuals, with a focus on timely return to work.

You will collaborate with patients, physicians, and employers, manage assessments and plans, and ensure compliant, compassionate care while maintaining high-quality outcomes and adherence to HIPAA and state laws.

Qualifications

• RN licensure in the operating state.
• Fluency in English (speaking, reading, writing).
• Three or more years of recent clinical experience.

Responsabilités

• Provide medical case management with coordination among patient, physicians, and employer.
• Utilize Case Management steps: assessment, planning, implementation, evaluation, and outcome.
• Evaluate treatment plans for appropriateness, medical necessity and cost-effectiveness.
• Coordinate delivery of durable medical equipment and home health services.
• Assess rehabilitation facilities and transportation needs; coordinate home modifications if needed.
• Communicate medical information clearly and compassionately to patients and families.
• Stay current with health-care laws (HIPAA, FMLA, ADA, STD, LTD, SSDI).
• Utilize technology to prepare timely reports compliant with HIPAA.
• Research medical and community resources for catastrophic or chronic diagnoses.
• Possess a valid driver's license and travel up to 90% of the time.
• Perform other duties as assigned.
