Job summary

Qualifications

• Minimum one year of palliative care experience.
• CAPC palliative care training completed.
• Bachelor's degree in Nursing or related field AND minimum three years of nursing/case management experience.
• Registered Nurse License (California) required at hire.

Responsibilities

• Builds effective relationships by providing resources and mentoring; communicates feedback and adapts to change.
• Works autonomously using up-to-date subject area expertise to solve problems and follow procedures.
• Performs initial case assessment interviews and determines eligibility and education for admissions.
• Monitors and evaluates care plans; coordinates resources and updates authorizations as needed.
• Produces case-management plans with goals, collaborating with patient and care team.
• Keeps knowledge current with seminars and research; shares resources with the team.
• Develops comprehensive disposition plans in collaboration with patient, family, and providers.
• Connects patients to services and external resources with minimal guidance.
• Acts as liaison between internal/external care teams and vendors; supports problem solving.
