Qualificações

• Bachelor’s degree in Nursing from an accredited school of nursing and three years work experience as a Registered Nurse required.
• Prior leadership experience preferred.
• One year of Case Management experience preferred.
• Case Management certification preferred.

Responsabilidades

• Coordinate a plan of care to achieve clinical outcomes and promote safe, timely discharge.
• Manage the plan of care across the care continuum to ensure efficient patient movement through the hospital system.
