Job summary

The Nurse Case Manager II reports to the Manager of Case Management and Care Transitions and works under the guidance of the Nurse Case Manager Team Leader. This role functions as a clinical team member, coordinating a plan of care to achieve clinical outcomes and promote a safe and timely discharge throughout the care continuum.

This position serves as a clinical expert in discharge planning, resource utilization, and case management, ensuring efficient patient flow through the hospital system.

Qualifications

• Bachelor's degree in Nursing from an accredited school of nursing and three years work experience as a Registered Nurse required.
• Prior leadership experience preferred.
• One year of Case Management experience preferred.
• Case Management certification preferred.

Responsibilities

• Coordinates a plan of care to achieve clinical outcomes and promote a safe and timely discharge.
• Manages the plan of care across the care continuum to ensure efficient movement of the patient through the hospital system.
• Serves as a clinical expert in discharge planning, resource utilization, and case management.
