The Senior Nurse Care Manager is a leadership role in a remote capacity, focused on providing high-level telephonic care coordination for high-risk patients with complex chronic conditions. This role includes mentoring, guiding, and overseeing a team of Nurse Care Managers, ensuring tailored care plans, and smooth care transitions. The Senior Nurse Care Manager will lead care coordination efforts, work closely with the multidisciplinary Care Team Pod, and act as an expert resource for less experienced staff.
Additionally, the role involves working with management to optimize workflows, identify improvements, and implement best practices. The Senior Nurse Care Manager serves as the main contact for escalated patient issues and plays a key role in meeting organizational care coordination goals.
