Job description

Location : Remote: Source from McAllen, Edinburg, Pharr, Mission, and Weslaco area

Shift : M-F, 1 pm - 9 pm CST

Job Description: Position Purpose :

Performs care management duties to assess and coordinate all aspects of medical and supporting services across the continuum of care for complex/high acuity populations with primary medical/physical health needs to promote quality, cost-effective care. Develops personalized care plans for long-term care members, addresses issues, and educates members and their families/caregivers on available services and benefits to ensure appropriate, high-quality care.

Education / Experience :

Requires graduation from an accredited nursing school or a Bachelor's degree with related experience. A Bachelor's degree in Nursing is preferred.

License / Certification :

RN - Registered Nurse with state licensure and/or compact licensure required. Resource Utilization Group (RUG) certification required at the time of hire.

Day-to-Day Responsibilities and Project Description :
  • Transcribing assessments and following up with members, completing assigned tasks.
Performance Expectations / Metrics :
  • Support medical, behavioral, and social support staff to determine needs, eligibility, and services for members.
  • Conduct screenings and risk assessments per program guidelines.
  • Identify members with special needs through health screens and other resources.
  • Coordinate member care with community outreach and member advocates.
  • Educate providers and community resources on program components and support services.
  • Educate members with special needs to foster compliance and improve outcomes.
  • Conduct site visits and support other special programs as needed.
  • Assist with care plan modifications and management, involving caregivers, DFPS caseworkers, and physicians.
Required Skills / Experience :

RN license, computer skills, and ability to work independently. Experience in home health is preferred.

Additional Information :

Resource Utilization Group (RUG) certification required at hire.
