Job summary
US Career Partners is recruiting an experienced Nurse Practitioner or Physician Assistant to deliver outpatient and SNF-based palliative care across Essex and Hudson counties in New Jersey. The role includes a structured ramp into palliative care with physician/APP backup and a supportive interdisciplinary team.
The position offers a base salary of $130,000–$160,000 per year, a bonus up to 12.5%, a Monday–Friday schedule, generous PTO, and comprehensive benefits including medical, malpractice
Qualifications
- NJ NP/PA license with national board certification.
- 2+ years of palliative, hospice, or serious-illness care experience.
- DEA + CDS registration or ability to obtain by start date.
Responsibilities
- Provide outpatient and SNF-based palliative care across Essex & Hudson counties.
- Lead goals-of-care discussions with patients and families.
- Coordinate with home health and hospice agencies as appropriate.
- Document timely and compliant clinical, billing, and quality data.
- Travel locally within Burlington County with mileage reimbursement.
