Job summary

NJ Palliative Care Solutions is hiring an experienced Nurse Practitioner or Physician Assistant to provide outpatient and SNF-based palliative care across Essex & Hudson for patients living with serious illness.

This is our experienced-provider track: 2+ years of palliative, hospice, or other serious-illness care is required, with mentorship, clinical backup, and data-guided coaching to grow.

Qualifications

• 2+ years of palliative care or closely related serious-illness experience.
• Active New Jersey NP or PA license and national board certification.
• DEA + CDS registration, or ability to obtain by start date.
• Palliative care certification preferred (e.g., ACHPN / CHPN).

Responsibilities

• Provide outpatient and SNF-based palliative care with a patient-centered approach.
• Lead goals-of-care discussions and coordinate with home health and hospice when appropriate.
• Deliver comprehensive palliative assessments and follow-up visits across SNFs and community settings.
• Partner with interdisciplinary team to ensure smooth transitions and coordinated care.
• Complete timely documentation aligned with clinical, billing, and quality standards.
