Job summary
Optum is seeking a Nurse Practitioner or Physician Assistant to provide longitudinal, complex care for medically complex patients in skilled nursing facilities across Lane County, Oregon. This field-based, full-time role focuses on high-quality, patient-centered care within a value-based model, collaborating with interdisciplinary teams to improve outcomes and reduce unnecessary utilization.
The ideal candidate will have NP or PA credentials, state licensure, DEA, and board certification, with
Qualifications
- Requires NP or PA licensure in home state or ability to obtain by start date
- Board certification through AANP/ANCC or NCCPA as applicable
- Active DEA licensure or prescriptive authority or ability to obtain post-hire
Responsibilities
- Conduct comprehensive assessments for medically complex patients
- Manage acute and chronic conditions with diagnostics and treatment
- Provide care during sub-acute rehabilitation episodes
- Develop and oversee complex treatment plans
- Lead goals-of-care and advance care planning discussions
- Coordinate with primary care, facility staff and interdisciplinary team
- Educate patients and caregivers on treatment plans and disease management
- Ensure accurate clinical documentation
