Job summary

UnitedHealth Group's Optum Home & Community Care is hiring a Senior Advanced Practice Clinician for the Senior Community Care program to coordinate care in nursing homes and senior living settings.

We offer a sign-on bonus, no on-call, weekends, or holidays. The role emphasizes comprehensive assessments, treatment planning, and collaboration with the interdisciplinary team to improve access to care for high-risk members.

Qualifications

• Active NP certification in Family/Adult/Geriatric/Adult-Gerontology Primary Care.
• Unrestricted state license and ability to practice without disciplinary actions.
• DEA licensure or prescriptive authority, or ability to obtain post-hire.
• Ability to obtain a collaborative practice agreement where applicable.

Responsibilities

• Perform comprehensive assessments including history, exam, and documentation.
• Support members during short-term sub-acute rehabilitation episodes.
• Maintain accurate documentation including ICD-10 conditions.
• Collaborate with the interdisciplinary care team.
• Conduct advanced illness and end-of-life planning conversations.
• Provide counseling and education to patients and caregivers.
