Job summary

Qualifications

• Master of Social Work (MSW) from a CSWE-accredited program.
• Licensed Clinical Social Worker (LCSW) in Virginia required.
• Experience with behavioral health assessment, crisis intervention, and discharge planning in acute care setting.

Responsibilities

• Perform comprehensive psychosocial, behavioral health, and risk assessments; develop intervention plans; provide crisis intervention and brief counseling.
• Coordinate referrals to behavioral health, substance use, and community resources to support transitions of care.
• Lead and mentor junior social workers; participate in interdisciplinary rounds and quality improvement initiatives.
• Document assessments and progress in the electronic medical record per standards.
