Job summary

Qualifications

• High school diploma or GED required.
• Certification and Iowa Board registration required at time of hire, or licensed pharmacist-intern status.
• Minimum 2,000 hours of pharmacy technician experience.
• Ability to interview patients and caregivers to obtain medication histories.
• Ability to record allergies and medication lists in the EMR and source documentation.

Responsibilities

• Obtain and verify accurate patient medication histories.
• Interview patients or caregivers to capture current medication regimens.
• Record medications, allergies, and sources in the EMR.
• Consult with a pharmacist on unresolved questions regarding history or allergies.
• Support pharmacy team in routine medication history processes.
