MercyOne North Iowa in Mason City is seeking a Medication Historian Pharmacy Technician to support accurate medication histories at hospital admission. You will interview patients, document prescriptions, OTCs and supplements, and note allergies to safeguard patient safety.

The role requires a high school diploma, Iowa tech registration or eligibility, and national certification within 6 months. Expertise with interviewing, data gathering, and compliant documentation is essential.

Qualifications

• A high school diploma or equivalent is required.
• Must be a registered pharmacy technician or be eligible for technician/trainee registration in the State of Iowa.
• National Pharmacy Technician Certification required within 6 months of employment.

Responsibilities

• Obtains an accurate and complete admission medication list via face-to-face patient interviews.
• Documents prescriptions, OTCs, and supplements with dose, frequency, and last dose taken.
• Notes known drug allergies and reactions; may collaborate with family, physician, or other sources.
• Communicates professionally and uses interviewing skills to obtain medication histories for diverse patient populations.
• Applies advanced computer skills to gather information and document admission medications.
