Job summary

Riverview Jefferson in Detroit, MI seeks an experienced MDS Coordinator (LPN or RN) to join the team on the day shift. This role focuses on resident care planning, MDS/MMQ processes, and ensuring compliance with state, federal, and accreditation standards.

You will serve as a resource for care planning, complete MMQ turnaround documents promptly, support program development, participate in facility meetings, and promote effective communication among staff, physicians, and residents.

Qualifications

• Must have knowledge of MMD/MMQ rules and regulations.
• Nursing degree and MI license required.
• Strong assessment, communication, and leadership abilities.

Responsibilities

• Assist in resident care planning and MDS/MMQ processes.
• Serve as a resource for care planning activities.
• Complete MMQ and turnaround documents promptly and communicate with relevant departments.
• Ensure compliance with state, federal, regulatory, accreditation, and reimbursement guidelines.
• Participate in quality improvement activities and staff development.
• Promote clear communication among staff, physicians, residents, and families.
