Job summary

Qualifications

• Bachelor's degree in health discipline or equivalent.
• Current State Licensing as an LPN or Nurse with 2+ years in long-term care.
• English communication skills (written and verbal).
• First Aid / CPR Certification.
• Background check and pre-employment screenings (TB, drug screen, etc.).

Responsibilities

• Coordinate and manage the MDS data collection process.
• Assess MDS forms for all residents and schedule annual assessments with Nursing leadership.
• Review past survey results to identify areas for improvement in MDS.
• Complete nursing section of MDS assessments and transmit materials to the State.
• Maintain knowledge of RAI/MDS laws and policies.
• Communicate resident information clearly to staff and management; interview residents/families as needed.
• Participate in admissions, Medicare, and quarterly care conferences.
• Assist in resident care and evaluations as needed.
• Perform other duties as assigned.
