Virginia Mason Franciscan Health is seeking a Masters Social Worker to provide compassionate advocacy and social work support for patients and families facing complex challenges in a hospital and emergency department setting.

The role emphasizes crisis intervention, care coordination, and linkage to community resources, with a focus on safe discharge planning and patient-centered care across the Puget Sound region.

Qualifications

• Master's degree from a school of social work accredited by the Council on Social Work Education (CSWE).
• 1-year post-MSW clinical or medical setting experience or social work internship.
• Licensed Social Worker Associate Independent Clinical (LSWAIC) or Licensed Advanced Social Worker or Licensed Social Worker Associate Advanced or Licensed Agency Affiliated Counselor.

Responsibilities

• Perform social work assessments and interventions for hospitalized and emergency department patients.
• Provide crisis intervention and patient/family advocacy.
• Coordinate and facilitate family/patient meetings with the health care team as needed.
• Arrange post-acute placement and engage community resources for high-risk patients.
• Contribute to risk management and patient/family education.
