You will collaborate with Operations and Clinical leaders, monitor utilization trends, and drive initiatives to enhance quality and efficiency while aligning with value-based care strategies.

Qualifications

• 7+ years in care coordination, case management, utilization management, or population health.
• Leadership or supervisory experience in healthcare settings.
• Experience analyzing healthcare data to drive operational improvements.

Responsibilities

• Lead, coach, and develop a team of Referral Coordinators; foster accountability and member-centered service.
• Oversee performance through hiring, onboarding, coaching, and evaluations.
• Monitor metrics, drive engagement, retention, and career growth.
• Develop and optimize standardized referral coordination processes and best practices.
• Oversee referral utilization and network performance using data to guide cost-effective care.
