The ideal candidate will guide performance, leverage analytics, and drive improvements in network utilization and care coordination outcomes while partnering with market and enterprise leaders.

Qualifications

• 7+ years in care coordination, case management, utilization or population health.
• Leadership or supervisory experience in healthcare.
• Experience with referral management workflows and utilization trends.
• Ability to translate data into operational improvements.
• Wamiliarity with value-based care models preferred.

Responsibilities

• Lead, coach, and develop a team of Referral Coordinators.
• Manage performance via onboarding, goals, coaching, and evaluations.
• Monitor metrics and drive engagement, retention, and growth.
• Develop and optimize standardized referral processes and tools.
• Oversee referral utilization and network performance with data-driven insights.
• Provide reporting and recommendations to leaders for decision-making.
• Ensure adoption and optimization of referral management systems and workflows.
• Collaborate with Market Leadership, Operations, and Clinical partners.
