Job description

Title: Senior LTSS Service Care Manager (RN)

Duration: 06 Months (Potential extension/conversion)

Location: Remote - Laredo, TX (Member Facing)

Responsibilities:

This position is for the Laredo area to complete assessments in members' homes as well as complete documentation according to State and Company guidelines to ensure compliance.

M-F visits 3 days a week, with OT possible with manager approval, documentation days 2 days a week, traveling to members' homes to complete assessments and documentation.

Face-to-face assessments are completed 3 days a week, with 2 documentation days, completing tasks, submitting all forms/assessments within turnaround times, and completing outreaches.
