Clinical - Senior LTSS Service Care Manager (RN)

Pay Range: $40 - $45/hour. Benefits include health insurance (medical, dental, vision), 401(k), and paid sick leave (depending on work location).

Key Responsibilities:

• Completing assessments and biannual reviews
• Care coordination and follow-up on member/LAR requests
• Completing required documentation and submitting authorizations
• Travel for home assessments
• Maintaining at least 92% audit score
• Providing education to members/LARs
• Preparing productivity reports
• Following up on HEDIS measures
• Assisting members/LARs with healthcare provider arrangements

Key Requirements and Technology Experience:

• RN license required; pediatrics experience is a plus
• Experience with Outlook, Excel, Word, Microsoft Teams
• Graduate from an accredited nursing program or a bachelor's degree with 4–6 years of related experience
• Bachelor’s degree in nursing preferred
