Job summary

Signature HealthCARE is seeking a dedicated LPN/LVN to provide hands-on nursing care in our Winchester, KY facility. You will administer medications, support ADLs, and supervise nursing staff while upholding safety and quality standards.

Qualifications

• Active LPN/LVN license in the state of practice (KY).
• Minimum 1 year of clinical nursing experience, long-term care preferred.
• Experience in supervisory or charge nurse role preferred.
• Strong observation, med administration, and documentation skills.
• Familiarity with EHR systems and Microsoft Office.

Responsibilities

• Provide direct nursing care to residents, including medication administration and wound care.
• Assist residents with ADLs ensuring comfort, dignity and safety.
• Supervise nursing assistants and medication aides during shifts.
• Monitor residents’ status and notify physician/others of changes.
• Develop and review individualized care plans with the interdisciplinary team.
• Complete clinical documentation per policy and procedure.
• Coordinate admissions, transfers and discharges.
• Participate in quality assurance, staff education and surveys.
• Perform other duties as assigned by the Director of Nursing.
