Job summary

Chesapeake Regional Healthcare is seeking a Social Worker Senior Case Manager (LMSW) to coordinate patient care across the continuum. You will apply discharge planning principles, collaborate with a multidisciplinary team, and leverage community resources to ensure safe, timely, and cost-effective transitions of care.

The role includes co-managing caseloads with RN Case Managers, addressing social determinants of health, and ensuring documentation in data systems.

Qualifications

• Minimum of three (3) years of clinical social work experience with two (2) years of case management experience.
• LMSW required; CCM/ACMA certification preferred.
• CPR certification within 6 months and maintain CPR.
• Proficiency in electronic health records (EHR).
• Knowledge of CMS regulations, payer guidelines, and accreditation standards (DNV, TJC).

Responsibilities

• Create and maintain discharge plans for admitted patients, considering psychosocial, functional, and safety status.
• Collaborate with physicians, nurses, and the care team to ensure appropriate patient management.
• Co-manage caseloads with RN Case Managers.
• Identify and address social determinants of health (SDOH) needs and connect to community resources.
• Monitor length of stay and resource use; identify delays and optimize discharge efficiency.
• Communicate resource utilization and discharge barriers to the team; follow policy.
• Document discharge plans in data systems per policy.
• Support post-acute activities and denials/approvals with relevant stakeholders.
• Serve as a mentor/preceptor for new hires where assigned.
