Freedom Square of Seminole is seeking a Social Services Coordinator to address medically related emotional and social needs of residents, acting as the licensed Social Worker for the Health Center. The role requires coordinating admissions, transitions, and care plans while ensuring HIPAA-compliant documentation and ethical standards.

You will work within an upscale senior living community, collaborating with families and multi-disciplinary teams to support residents across independent living,

Qualificações

• Bachelor's degree in social work or related field.
• Licensed Social Worker in the state of residence.
• Minimum 2 years of healthcare experience working with residents and families.

Responsabilidades

• Develops a comprehensive social history and assessment for residents, including social, religious, and cultural needs.
• Coordinates Health Center admissions and assessments.
• Guides residents and families through transitions and care planning.
• Assists in care plan development and attendance of care planning conferences.
• Maintains confidentiality and proper documentation in accordance with regulations.
