Job summary

Freedom Square of Seminole, an Upscale Senior Living Community in Seminole, Florida, is seeking a Social Services Coordinator to ensure residents' emotional and social needs are met and coordinated with the Health Center. As the licensed Social Worker for the Health Center, you will develop social histories, assist with admissions and transitions, and participate in care planning conferences.

Qualifications include a Bachelor's degree in social work or related field and licensure; the role

Qualifications

• Bachelor's degree in social work, Gerontology or related field.
• Licensed Social Worker in current state of residence.
• At least 2 years experience in a healthcare setting working directly with residents and families.

Responsibilities

• Develops a comprehensive social history and assessment for residents to inform care plans.
• Assists with Health Center admissions and assessments.
• Assists with transition for residents/families and orientation to facility, services, and rights.
• Participates in care plan development and attends care planning conferences.
• Ensures completion and maintenance of advanced directives in resident charts.
• Maintains confidentiality and documents in accordance with HIPAA and ethics.
