Freedom Square of Seminole seeks a Social Services Coordinator to address residents’ emotional and social needs and guide them through care transitions in line with Federal and State guidelines.

As the licensed Social Worker for the Health Center, you will develop care plans, support admissions, coordinate discharges, and collaborate with families to ensure a compassionate, compliant aging-in-place experience. Formación

• Bachelor's degree in social work, Gerontology or related field.
• Must be a licensed Social Worker in current state of residence.

Responsabilidades

• Develops a comprehensive social history and assessment of the resident.
• Assists with coordination of Health Center admissions and assessments.
• Assists resident/family with transition to the new environment and orientation to facilities.
• Participates in care plan development for Social Services component.
• Attends care planning conferences and facilitates resident/family participation.
• Ensures advance directives are complete and maintained in charts.
• Completes mandatory documentation per regulations.
• Maintains confidentiality of admissions information and resident records.
• Coordinates discharges and transfers with multidisciplinary staff.
