Freedom Square of Seminole is a Life Care Community in Seminole, FL seeking a Social Services Coordinator to meet residents’ emotional and social needs and support the Social Services Director. You will work within HIPAA guidelines and help plan care while coordinating with the Health Center team.

Requirements include a bachelor’s degree in social work, gerontology, or a related field and 2+ years in healthcare, preferably LTC.

Qualificações

• Bachelor's degree in social work, Gerontology or related field preferred.
• At least 2 years of experience in a healthcare setting (preferably Long-Term Care) working directly with residents and families. 1 year preferred of Social Services experience in LTC.
• Computer literate; PCC experience preferred.

Responsabilidades

• Develops a comprehensive social history and assessment of the resident including problems, strengths, and preferences to inform the care plan.
• Assists with coordination of Health Center admissions and assessments.
• Helps residents/families transition to the new environment and orientates them to the facility, services, limitations, and rights.
• Participates in the care plan development for Social Services components and attends care planning conferences.
• Ensures Advance Directives are complete and maintained in resident charts.
• Completes all mandatory documentation in a timely manner per regulations.
• Maintains confidentiality of admissions information and resident records in HIPAA-compliant manner.
• Coordinates discharges/transfers and develops discharge plans with residents/families and staff.
